Artifact Donation Form
If you are interested in donating or loaning an item or collection to us please fill out one of the forms below. Because of space and care requirements, we must consider every donation very carefully. Such a decision may be affected by the quality of the artifact, whether we have similar or identical artifacts, and whether we can assume the financial cost and storage space required to care for the artifact. We will review your answers to the questions and contact you with our decision.

Shelby House Usage Agreement
